About Large-Volume Liposuction
Large-volume liposuction specifically focuses on removing large amounts of excess fat while taking care not to disturb surrounding tissues. A common reason individuals seek this procedure is that traditional diet and exercise methods do not always address resistant fat pockets. “Large volume” generally refers to 4 to 5 liters (1 to 1.3 gallons) of fat for removal in one session. Liposuction is not a weight loss treatment itself. It is important that patients still practice healthy dieting and exercise habits after their procedure. However, liposuction can be an effective way to reduce stubborn pockets of fat for a more contoured appearance.

Patients with asthma, sleep apnea, high blood pressure, or pulmonary problems may still be acceptable candidates if the condition is well-controlled and under supervision by their primary physician. Patients must have a good attitude and realistic expectations of the result of their procedure. While large liposuction does provide immediate improvements, it takes time for the final results to be fully realized.
Personal Consultation
The first step in your large-volume liposuction procedure is a personal consultation with Dr. Younai at California Center for Plastic Surgery. At this appointment, Dr. Younai will assess your goals and medical history to create an optimal treatment plan. He will conduct a thorough evaluation of your body, discussing target areas for fat removal as well as examining your skin quality. Photos may be taken to monitor progress, and expectations regarding the recovery process, results timeline, and post-operative care instructions will be set forth. Your safety and satisfaction are the utmost priority, and recommendations will be made based on your assessment.
Preparation
A medical evaluation is performed on all prospective patients to ensure their safety. Following this, you will receive specific preparation instructions that are tailored to your medical history and procedural goals. These vary for each patient, but most can plan to:
- Adjust their medication use to avoid blood-thinning substances
- Avoid alcohol for a few weeks before their procedure
- Cease any form of nicotine use, especially smoking
- Arrange transportation to and from their procedure
- Preemptively fill any prescriptions provided by Dr. Younai
Procedure
Large-volume liposuction procedures begin with anesthesia administration. Patients will receive general anesthetic for this procedure and will be constantly monitored by a board-certified anesthesiologist during their surgery and recovery time.
Dr. Younai performs liposuction with the tumescent method, also known as “wet” liposuction. In these procedures, a tumescent solution containing epinephrine, lidocaine, and saline is injected into the tissue after the initial incisions are made. (2) The tumescent method infuses large amounts of diluted lidocaine, which is an anesthetic, and epinephrine, which is a vasoconstrictor. The use of this method has two main benefits; the lidocaine stays in the system after surgery, reducing the post-operative pain by numbing tissue from the inside, and the epinephrine constricts the capillaries in the fat layer, minimizing blood loss. (2) The use of tumescent fluids also allows for more tissue to be removed during one procedure, helping patients see impressive results in a shorter timeframe.
After incisions are made at the targeted areas and tumescent fluids are injected, Dr. Younai will carefully insert a small tube called a cannula. Dr. Younai will break down fat and remove it from the body with medical suction. The tumescent technique significantly decreases a patient’s recovery time and offers many advantages for surgeons. Patients can expect to see significant results after just one session. (2)
Recovery
An overnight hospital stay is generally recommended for large-volume liposuction patients. Monitoring these patients’ conditions after significant tissue removal is beneficial, and any complications like pain and nausea can be easily managed by our providers. Some patients may be able to return home after their procedure. Dr. Younai will assess the need for a hospital stay depending on the extent of the surgery and the overall health of the patient.
All patients will see Dr. Younai 24 to 48 hours after their procedure for an evaluation and to receive recovery instructions. Recovery depends on many factors that are unique to each patient and the extent of their procedure. Patients will experience post-operative swelling and bruising which diminishes over time. Most patients will be advised to wear a compression garment to minimize swelling. Keeping incisions clean and covered will also be important. It is generally recommended that patients avoid any exercise or intense activities for at least 2 weeks, or until they are advised to resume by Dr. Younai.

What happens if I gain weight after large-volume liposuction?
